Professional Portal

The Professional Portal can be used to complete all aspects of the Educational Health and Care (EHC) needs assessment and review process. Currently this is a new functionality which is only available for professionals to submit a request for an EHC needs assessment.

You can use this portal to track a young person’s progress through the Education, Health and Care (EHC) needs assessment and review process. The implementation of both portals will be a staged process and although the portal is a two-way communication tool this will initially only be used as a one-way communication until this is established. However, we do hope to share any documentation for children and young people being considered at SEND panel with representatives once forms have been submitted on the portal. You will receive more information regarding this if you are identified as a representative for the panel meetings.

In order to complete a submission for an EHC Needs Assessment Request you will need to register on the Professional Portal. You can access the Professional Portal here.

Document 1a REQUEST STAGE - Support Plan Overview (Ranges 1 & 2)

Document 4a - REQUEST STAGE - Guidance for EHC Needs Request Meeting

The EHC Needs Request Meeting is a key step in the process and this guidance document is to help you ensure that a multi-agency person centred approach is used, so an holistic view of the child or young person’s special educational needs can be gathered in co-production with children, young people and their families. 

Document 6b - REVIEW STAGE - Template- Annual Review Form

This must be used to capture information discussed at the EHCP review meeting and reflect the changes/amendments and recommendation of the review.  It is essential that all sections are completed and that there is a clear indication of where there are changes to be made, progress of outcomes and the recommendation from the review meeting.
